Add on hydraulic uses a hydraulic ram at the drive to assist the cable driven steering mounted internally to the steering tiller, but the cable still runs the show. A full hydraulic has a hydraulic helm and steering is done only on the exterior of the boat at the drive.

I've had add on dual ram on a single and now have full dual ram on twins. I would never want to go back to an add on system. It was very stiff to turn at idle with a single, so I'm guessing turning twins would be even harder to turn.
