Default OMC King Cobra "popping"out of gear

My buddy has a 1992 221 Lib with a 454/King Cobra set up. It has popped out of gear 3-4 times the last couple times out. Always when putting some power to it.

The first time it was when he landed power on in some big water(I know, not good)

The other times have been when getting on plane.

No clucks, grinds, noises of any kind and no metal in the lube. He simply puts it back into neutral, shifts it back into gear and it's fine. I know some KCs had shift cable issues. Could this be a cable issue and the boat isn't FULLY being shifted into gear? Then when he puts power through it it pops into neutral?

Yes that's a cable issue.
