Default Broken Torsion Axle

Is it unusual for a Torsion Axle to break on a 2 year old trailer? If not, what would be some of the primary causes for it to break?

I thought it was unusual too but a friend broke one on a ski boat trailer last week. It is unrepairable and axle has to be replaced but is only 150$ for complete axle from the axle manufacturer.

I lost a wheel coming home from the Chicago PR and have been trying to figure out why the lugs snapped off. I didn't notice the bent axle until yesterday. I've had the trailer since it's been new never been abused. Just concerned about the integrity of the trailer. Luckily it happened less than 40 miles from home. Never did find the wheel.

First two are the pics from underside of axle. The orange is rust where the paint has flaked. The arrow is pointing to the position of the axle where it bent. It's a aprt of the axle where it has been pressed earlier to create a factory bend (That's why there's a large "divit" in the metal-The bend is the bulging part). Left side is beginning to bulge as well....
