Go to the USCG documentation web site and download the form(s) that you need. Pretty simple.
When I traded the PQ, Active Thunder provided a Bill of Sale for me to sign. I guess it never got sent in cause I continued to get renewal forms. I called USCG and they said all I needed to do was write SOLD on the renewal form and fax it back to them.
