Everything is line of sight, except if you want to go to Anegada (sp). Do not do that trip in a sailboat, it takes waaay too long! 
There is an OSO member that owns a place in the Bitter End called Saba Rock. It got wiped out from the last hurricane and I believe that they are rebuilding. Not sure if it is done yet. l that guys name is David (OSO name-ExPat) and his GF's name is ChrisAnn.... Both were very nice people!

We did an awesome trip down there a few years back. 40 people, 5 boats...….and charts that were basically just a placemat.
