That was going way to fast for auto pilot

what would have prevented him from running into another boat even if it hadn't failed?

It is the law to keep a lookout. Remember the incident in NJ(?), couple/few years ago, when 62'r ran across the fishing boat w/ 4 NY business guys on board ... I think 3 were killed, major controversy about whether or not there was a lookout. Captain says there was ... NO WAY .. law suts ... settled out of court for major $$$$ Forget the 62' vessel's name??
