I have a copy of the form you need for the rebate in Virginia. If you give me a fax number I will fax it to you. I live in MD also but try to buy gas on the OTHER side of the river and do one form after each season. You must submit the form + receipts within one year of puchase and the receipts must state the number of gallons purchased. Always end up getting back a couple hundred bucks.
