how bad is this....
 
Port motor right side picture up the tail pipe. How bad is this and how hard to fix it.

Picture is blurry but it looks like just a broken flapper to keep water out? If so that is an easy fix. You can get kits that have a new flapper and pin. Just grind/drill the old pin out and have the new one welded in place.

Is it the flapper? If so it looks like the rubber has worn off(holes showing on bottom portion and what looks like rubber peeling) so it would be a good idea to replace it anyway. May have lost the sprng and wedged itself somehow so it won't move? I'd check all the others as well.

I put Salisbury external flaps on mine. Not exactly "eye candy" but they don't blow out or fall apart.

Is it in the tailpipe or in the exhaust tip? Usually the flappers are in the tip.

There in the tip. So what do I do just unscrew the screws on the outside of the transom and pull the tips out?

I got rubber hang down flappers on the external aswell

Yes, but be careful if 5200 was used as sealant you want to use a heat gun and slowly heat up the ring that has the screws that hold the tip on. If you muscle it off gel coat will come with it!!!

Also you will have to unclamp/unhook the tip where it attaches to the tailpipe, header, riser depending on the exhaust system you have. Mine just has a rubber hose going from the header to the tip.
