Weird Brake Pad Problem...

So I replaced the brake pads on my 03' myco about 6 weeks ago and have a 1200 miles on them. Yesterday I noticed a squeek coming from the left side. I take a peek and find the actual steel plate that the pad is molded to rubbing on the brake disc... never have I seen this before. All was done like I normally do... lube the pins, lube the slides, ect... what gives.. ideas?
Also, I can take the caliper when its tight and rock it to make the pad hit the rotor where you see the wear... that should not happen. Ugh!?!

I don't know about the rocking. That sounds like something is cracked on the mounting plate. All of them, or just one?
However, I HAVE seen this interference problem with Kodiak disc brakes before. On my last trailer upon doing a brake job, I had this same interference. I called Kodiak and they said to grind down the steel edge of the pad so it does not hit. It wasn't a big deal and worked fine ever since.

Its grooving every rotor... I pulled each pad and ground them to the pad mat'l thinking the same thing but no go. Thats when I pulled the pins and sliders and greased all of them again... and still no go. I plan on calling myco and kodiak today...
