Mechanical trim actuator, should be attached with stainless clamps.
The black rod is spring loaded and attached the the cable. If the stainless tube/guide (the one with zip ties) is not snug on the ram it will just slide.

So is that why there is that extra piece on the end of the trim rams?

If you put the drive down (assuming your indicator on the helm is in working order) and if the Zip ties can hold it in place you will see the black plunger contacts the Stainless fitting.... as the plunger is pushed in as the drive tucks in - the indicator moves accordingly.

Definitely put on the Stainless clamps (make sure Band and Screw is SS) to hold the cable in place on the ram

Some do but they are subject to corrosion and they aren't reliable thus the mechanical type are used.

it looks like you are missing the cradle that goes between the trim cylinder and the indicator tube. you have to have it to hold it off the trim cylinder at the correct angle and it has notches in it to locate the clamps. the spring in there is pretty stout so with out it clamped properly it will slide
