There are several ways to do this.
A standard chain drive and a reverse rotation cam, or a gear drive so that you can use a standard rotation cam.
Counter rotating cam requires a different distributor gear, some say rear main seal and a starter.
BW just needs the pump flipped they say.

It is my understanding that most if not all BW trans can not be ran in reverse. Maybe it is just the older ones? Reverse rotation engines are a rarity anymore. I am running twin disc gears and one is ran in reverse with older Arnesons. Seems to work fine.

The velvet drive can have the pump indexed to run standard or reverse rotation. That trans rotation has to be the same as the engine rotation, ie you can not run a velvet drive in reverse at full power. Most twin inboard engines are set up this way, one standard, one reverse rotation engine with trans pumps indexed accordingly.
The boat you're describing was probably setup prior to the widespead availability of transmissions like the Hurth, BW 5000 series, etc that can be run in both directions on a standard rotation engine.
And as Rik pionted out, some arnesons are availale with a drop box that is chain or gear driven that change he prop rotation. This allows for 2 standard rotation engines.
