The bungs should be 18mm same as most sparkplugs and yes 6'oclock is a lousy position. We put ours at 3 o'clock and 9 o'clock depending on which side is better to route the cable. The stainless 18mm bungs sold by most parts houses can be had with the corresponding plug. Where the bungs are placed will vary depending on the exhaust manifolds or headers, but as far back towards the engine as possible and still read all 4 cylinders on a side if possible. This usually keeps them away from water in the exhaust as much as possible and away from water/exhaust mixing points and captains call. etc.
Yes, we only use a wideband 02 sensor for air/fuels, conventional automotive non-wideband would be useless.
I think some of the Napa houses still catalogue the sparkplug anti-fouling adaptors and you are probably correct when you mention good ol J.C Whitney-Boy been a long time since I looked at on of their catalogues!
