I was wondering what is the ultimate collection of data to get before contacting a prop shop.

From what I see here.

Boat weight,
Hull type, Pad or step etc.
Motor HP and torque curve.
x dimension
drive type.
gps and rpm at wot
trim

anything else?

it would be nice to have a form we could fill, in maybe a pdf form we could print out and take with us.

Call Throttle Up and Julie or Matt will tell you what they need.

The make and pitch of the propeller used for testing.
Gear Ratio.
Testing conditions; water and weather.
Load of boat: ie; fuel, people and gear.
