Default How to test before calling prop shop.

I was wondering what is the ultimate collection of data to get before contacting a prop shop.

From what I see here.

Boat weight,
Hull type, Pad or step etc.
Motor HP and torque curve.
x dimension
drive type.
gps and rpm at wot
trim

anything else?

it would be nice to have a form we could fill, in maybe a pdf form we could print out and take with us.

Gear Ratio.
Testing conditions; water and weather.
Load of boat: ie; fuel, people and gear.
